Registration for the Muleshoe Community Outreach Screening is now open!

To register your child, please click here.

Please click here and download the OSYA Medical Questionnaire. We ask that you complete each page of the packet to the best of your knowledge based and bring the packet to the screening. This will help speed up the registration process.  If you have more than one child attending, you will need to fill out a packet for each child.

Remember your child MUST be between the ages of 12-18 years old.
We cannot allow exceptions to this age limit for any reason.

A parent MUST attend the screening with the youth.

No caffeine for 24 hours before the screening. Caffeine can affect the results.

Bass Tournament Rules and Regulations

OSYA is still actively looking for sponsors for the bass tournament and our organization. For a minimum of $100, your sponsorship will get your name/company name on a sponsorship banner we will display at all of our events this year. We will also promote you on our website on a special page for bass tournament sponsors as well as all of our social media sites (Facebook, Twitter, and Instagram). All sponsorship is tax deductible and we are happy to share that information with you!
