Registration for the our next screening for Lubbock is now open!

Please click here and download the OSYA Medical Questionnaire. We ask that you complete each page of the packet to the best of your knowledge based and bring the packet to the screening. This will help speed up the registration process.  If you have more than one child attending, you will need to fill out a packet for each child.

Remember your child MUST be between the ages of 12-18 years old.
We cannot allow exceptions to this age limit for any reason.

A parent MUST attend the screening with the youth.

No caffeine for 24 hours before the screening. Caffeine can affect the results.

After much discussion, it has been decided this will be the last screening in Lubbock for 2018. Do not worry. We are not going to cease our screening efforts. We will continue to hold screenings yearly in Lubbock. However, we feel we need to put more focus on our community outreach to assist the outlying communities of West Texas that are unable to travel to Lubbock.
