Remember your child MUST be between the ages of 12-18 years old.
We cannot allow exceptions to this age limit for any reason.
To expedite the registration process and keep things running smoothly, we ask that parents bring the medical questionnaire filled out. Please click here to download the packet. Please answer the questions as thoroughly as possible.
Please do not allow your child drink ANY caffeine or energy drinks for 24 hours prior to attending the screening.

We have set aside a specific time slot during our Muleshoe screening for anyone from the Smyer ISD. This time slot is from 1:45-3:00 PM. If you are a part of the Smyer community and would like your child screening during this time, please Susan directly by clicking here.
