This will help speed up the registration process.  If you have more than one child attending, you will need to fill out a packet for each child.

Remember your child MUST be between the ages of 12-18 years old. We cannot allow exceptions to this age limit for any reason.

A parent MUST attend the screening with the youth.

No caffeine for 24 hours before the screening. Caffeine can affect the results.

Parents please note, Texas House Bill 76 signed by Gov. Greg Abbott on June 14, 2019 requires all Texas high school students participating in UIL events to have a cardiac assessment completed before they participate in any event. This law goes into effect September 1, 2019. By participating in our cardiac screening you will be compliant with this new law.

After much discussion, it has been decided this will be the last screening in Lubbock for 2018. Do not worry. We are not going to cease our screening efforts. We will continue to hold screenings yearly in Lubbock. However, we feel we need to put more focus on our community outreach to assist the outlying communities of West Texas that are unable to travel to Lubbock.

OSYA is continuing our mission to prevent sudden cardiac death in youth. We are taking our mission on the road again this year! We are happy to announce our next community outreach screening! We will again be visiting Muleshoe, TX and screening the youth of that area and eastern New Mexico.
