It takes a team effort to save lives. The founders of Operation Screen Your Athlete recognize this and invite you to join our team. You do not have to be a member of the medical community or have a medical background to join Operation Screen Your Athlete.

Membership Requirements:

• Must be at least 18 years of age
• Must pay a $10 annual membership fee
• Must fill out a membership application
• Must fill out a Code of Conduct
• Must fill out a Conflict of Interest disclosure

If you would like to become a member of Operation Screen Your Athlete, please download all 3 forms below. You must complete the forms completely and send them along with a check or money order for $10 to the address listed below.
